If you have PoTS, nobody needs to tell you to think about hydration.
You probably already do.
PoTS affects the body’s response to being upright and can be associated with symptoms including dizziness, palpitations, fatigue, brain fog, weakness and reduced tolerance of normal everyday activity.
For some people with PoTS, reduced effective circulating volume forms part of the picture. That is one reason fluid and salt strategies can play such a prominent role in day-to-day management.
Intravenous saline takes a different route.
Instead of relying on fluid passing through the digestive system first, saline is administered directly into the circulation.
Clinical research has found that acute IV saline can improve orthostatic tolerance and reduce symptoms in some people with PoTS.
So the interesting question is not simply, “Can IV saline affect PoTS symptoms?”

PoTS is not the same experience for everybody.
Different mechanisms can contribute to symptoms, and response to IV saline varies between patients.

What Does the Research Actually Show?
Interest in IV saline for PoTS goes beyond social media stories and patient anecdotes.
Clinical researchers have investigated what happens when circulating volume is increased using intravenous saline.
Acute IV Saline
Earlier research found that intravenous saline given acutely reduced symptoms during tilt testing in patients with PoTS.
Other research into acute volume loading found that IV saline improved resting haemodynamic measures and has previously been associated with reduced heart rate, improved orthostatic tolerance and symptom improvement, although it did not improve peak exercise capacity in that study.
Intermittent IV Saline
A 2017 observational study followed 57 people with medication-refractory PoTS receiving intermittent IV saline.
Participants reported improvements in orthostatic symptom scores and quality of life after beginning intermittent saline infusions.
The researchers suggested further investigation of intermittent saline as a possible bridge strategy for patients with high symptom severity.
Hydration & Orthostatic Intolerance
A later review examining oral and intravenous hydration strategies described water and salt intake as central to increasing intravascular volume in disorders that impair orthostatic tolerance and discussed intravenous volume loading in selected severe cases.
The evidence still has limitations.
Studies examining repeated IV saline have generally been relatively small, and important research has been observational rather than large randomised controlled trials.
So while there is evidence worth discussing, nobody should promise that IV saline will produce the same response in every patient with PoTS.

IV Hydration Is a Clinical Tool
PoTS is complex.
Different patients can have very different mechanisms driving similar-looking symptoms.

IV hydration can temporarily increase circulating fluid volume, but it does not remove the underlying cause of PoTS.

What About Repeat IV Saline?
This is where individual response and clinical judgement become particularly important.
Published observational evidence has reported improvements following intermittent IV saline in some highly symptomatic, medication-refractory patients.
However, repeated IV access has disadvantages and the current evidence does not establish one universal long-term IV saline schedule that is appropriate for every person with PoTS.
Repeated cannulation can contribute to:
- Bruising
- Vein irritation
- Infiltration
- Infection
- Thrombosis
- Increasing difficulty obtaining peripheral venous access
If you are interested in repeat private IV hydration, that should therefore be reviewed individually based on your circumstances, previous response, risks and wider management plan.
A previous successful infusion does not automatically mean the next appointment should happen without reassessment.

Can an IV drip cure PoTS?
No. IV saline can increase circulating volume and may temporarily improve orthostatic symptoms in some patients, but it does not remove the underlying cause of PoTS.
Can IV saline help PoTS symptoms?
Published studies have reported improvements in orthostatic symptoms following acute IV saline in some people with PoTS. An observational study involving medication-refractory patients also reported improvements in symptom scores and quality of life following intermittent saline. Individual responses vary.
Why might IV saline make a difference?
Saline increases intravascular fluid volume. In patients where reduced effective circulating volume contributes to their PoTS, increasing volume can temporarily influence heart rate, circulation and the response to standing.
Is IV hydration only for people who are severely dehydrated?
No single rule covers every patient. The clinical reason for IV fluid should be clear, and your symptoms, fluid intake, medical history and current circumstances need to be considered before treatment.
Can I book even if I already drink lots of water?
Yes, you can book a private consultation to discuss your circumstances. A high oral fluid intake does not automatically mean IV saline is appropriate, so your clinician will assess the wider picture first.
Do I need a confirmed PoTS diagnosis?
This service is designed for hydration assessment rather than diagnosing unexplained dizziness, tachycardia or fainting. If you have not been diagnosed with PoTS, significant or unexplained symptoms should receive appropriate medical investigation.
Can I have IV hydration during a PoTS flare?
You can book an assessment. A flare can have different triggers, so your clinician will review what has changed, your fluid intake, current symptoms and whether IV saline is reasonable in your individual situation.
Can hot weather make PoTS symptoms harder to manage?
Heat can be challenging for many people with orthostatic intolerance. Increased sweating and changes in circulation can make fluid balance more difficult for some patients.
What if I have vomiting or diarrhoea?
Vomiting and diarrhoea can cause substantial fluid loss and may make maintaining your normal oral intake difficult. Your clinical condition should be assessed, particularly if you cannot keep fluids down or are becoming increasingly unwell.
Can I have regular private IV hydration appointments?
Repeat treatment requires individual clinical review. Published observational evidence exists for intermittent saline in selected highly symptomatic patients, but repeated IV access carries risks and there is no universal schedule appropriate for every person with PoTS.
